Scheduling the District 33-A Lions Sightmobile

 

   The Sightmobile was dedicated on April 9, 2003. How about bringing the Sightmobile to your community? It's never too early to schedule a date, well almost. We're now accepting reservations up to one year in advance on a first come, first serve basis. There are many great dates left. You need to pick one now before someone else requests the same date first. Check the Sightmobile Schedule to see what dates might be available. The Sightmobile is available for screenings, parades and public relations. Rain dates may be requested but are not guaranteed. So act fast. Fill out a Reservation Form pdf and return it ASAP VIA US MAIL or EMAIL. To print it, make sure all margins are .25 inches.

  1. Email me or call ahead to see if your date is available. The schedule changes frequently. Contact an approved driver who is on the Sightmobile Drivers List in the District 33-A Directory before confirming your requested date.
  2. If you want to send the Reservation Form electronically, save it as a .txt file, edit it with the appropriate information, then save it. Attach the saved file to your email message to me.
  3. The Sightmobile is located at 55 Tanzio Road, Leominster Mass (directions). The keys to the Sightmobile will be at the Leominster Police Department, 29 Church Street, Leominster, MA 01453 (Police Station directions). Please make sure you have your driver's license and your Sightmobile driver identification card to present to the officer in order to get the keys.

NOTE THAT ALL REQUESTS MUST BE MAILED AS EMAILED APPLICATIONS ARE NOT COMPATABLE.

District 33-A Sightmobile Screening Information

To have a successful screening, we recommend the following:

  • Minimum of one Eye Doctor. Without a doctor, there can be no analysis/recommendations for referrals of the screening results.
  • Five support staff to run the 2 vision screening machines, tonometer, hearing booth and non-mydriatic retinal camera.
  • A driver who is familiar with the new Sightmobile and the sightmobile equipment (generator, levelers, etc.)
  • Two nurses/EMTs for testing for glucose and blood pressure.
  • Two people for signing in people at the door.

 In addition,

  • You need to advertise extensively
  • Do the screening at a school or senior center or town function to increase publicity and usage.

 Remember:

  • Absolutely no donation canisters or raffles.
  • Display information about Lions, your club, eye care pamphlets.
  • No membership drives permitted.
  • No smoking or drinking in the Sightmobile.
  • No attaching posters or banners on the coach - inside or outside.
  • There are only 2 seats (driver and one passenger) when transporting the Sightmobile.

                                 SUPPLIES NEEDED FOR A SCREENING FOR DISTRICT 33-A SIGHTMOBILE

 

Since each event may screen 50 - 100 people, supplies may be exhausted quickly.

  • Paper towels (preferably folded variety)
  • Alcohol-free, fragrance-free wipes (Baby wipes) to clean headrests and chinrests of Vision screening machines, Tonometer, Non-mydriatic Retinal Camera and Hearing Booth headphones after each person's use (Get Equate brand at Walmart. Refills are approximately $5.00 for 240 wipes.)
  • Hand sanitizer (Purell) for nurses
